Alternative N.V.A. Vision Program


Who is Eligible?

  • Retiree from a PFT bargaining unit
  • The spouse
  • Dependent children up to attainment of age 19
  • Dependent children up to attainment of age 25, living at home, unmarried, enrolled in a post-high school educational program
  • Incapacitated dependent children, 19 years or over provided the child was incapacitated before age 19  Back to Top

  • There are no limitations

Purpose

  • To provide the retiree with the ability to purchase eye care and glasses at a controlled price.
  • To provide thorough vision analysis (including Tonometry test), lenses manufactured to exacting standards and a wide selection of quality frames at a controlled cost.
  • To control the cost of more expensive frames and cosmetic items such as tints and photograys so that the member may save money. The savings could be as much as 50% of the retail cost.  Back to Top

If You Elect to Use One of the Participating Optometrists, Ophthalmologists, or Opticians in Southeastern Pennsylvania There Is a Basic Charge for:

  • examination...Up to $25.00
  • administrative fee...
  • any frames whose wholesale cost is $24.00 or less
  • regular lenses (two)...Up to $27.00
  • bifocal lenses (two)...Up to $38.00
  • trifocal (two)...Up to $47.00
  • lenticular (two)...Up to $64.00
  • a basic pair of regular-sized glasses, including the examination, could cost less than $83.00 (depending on the frames selected, with no extras)  Back to Top

Frames

  • The $24.00 basic charge is applied to the wholesale cost of the frames. These typically sell for approximately $48.00. Currently, you pay the retail price for all items!
  • If you pick more expensive frames you pay the difference between the $24.00 and the actual wholesale cost plus 20% of this difference.  Back to Top

Payments

  • Your basics, as listed above, are billed to you by the Fund. You pay Optometrist or Ophthalmologist for the extras. These maximum charges are listed on the form and checked by N.V.A.
  • All basic charges are billed to you directly from the Fund. Basic charges include the examination, regular-sized lenses, administration fee, and regular-sized frames (up to $24.00 wholesale price).
  • You pay the doctor directly for extras (prices listed and signed for by you when you ordered your glasses) when you pick up the glasses.
  • FAILURE TO PAY THE FUND BILL WITHIN 30 DAYS COULD RESULT IN THE RETIREE LOSING THE PRESCRIPTION BENEFIT.  Back to Top

How does an eligible person obtain the Special Vision Service Plan Form?

  • Obtain a Request for a Special N.V.A. Vision Form from the Fund office.
  • Mail the Request to the PFT Health and Welfare office.
  • You will receive, in the mail, a Special Form with the name of the requested eligible person, and a list of eligible providers. The form will also have an expiration date on it.
  • Choose a listed provider. Make an appointment for a date prior to the expiration date on the form.
  • After you order your glasses you will receive a bill for the basic portion of your order from the Fund office. Pay the bill within 30 days.
  • You pay the doctor for listed extras when you pick up your glasses.
  • Remember, failure to pay the Fund bill could result in loss of the Prescription Benefit.  Back to Top
